As the Teacher/Coordinator booking a Humane Classroom Kit, I understand the following: The above is only a booking request form and my booking will be confirmed once I am contacted by the Regina Humane Society via booking confirmation email. All Humane Classroom Kits must be picked up and returned to the Regina Humane Society on the dates and times indicated in my booking confirmation email. I am responsible for the Humane Classroom Kit requested during my booking and I may not lend the requested kit or any of its content/materials to other individuals/groups/organizations. I am responsible to notify the RHS of any content/materials that is damaged during my booking and ensure the requested kit is returned with all content/materials.(Required)
